New Client Forms – Mediation

The mediation process cannot begin without filing a completed and signed set of the following forms filed with my office. All parties involved in the mediation process are advised to bring completed and signed copies of these forms to the first mediation appointment.

Agreement to Mediate

This agreement form outlines the terms and conditions of the mediation process, including a fees and payment schedule. By signing this form you are agreeing to terms of the mediation, my responsibilities as mediator, your responsibilities and rights, and the payment of fees. My fees and payment terms are also summarized here.

Client Information Sheet

This information sheet provides basic information about yourself and the other relevant parties involved in the mediation.
